ABSTRACT

ntroducción: El diagnóstico tardío de la infección por VIH y el acceso de los pacientes con enfermedad avanzada al sistema de salud afectan negativamente los beneficios in-dividuales y colectivos del tratamiento antirretroviral. A nivel mundial existe una alta prevalencia de diagnóstico tardío es-pecialmente en poblaciones vulnerables como los migrantes.Objetivos: Medir la prevalencia de diagnóstico tardío de infección por VIH entre migrantes internacionales y com-pararla con la de los argentinos.Material y métodos: Estudio retrospectivo, observacional, de personas mayores de 16 años asistidas en el Hospital General de Agudos Donación Francisco Santojanni que hubieran recibido diagnóstico de infección por VIH entre 01/1/2018 y el 31/12/2021. Se determinó la mediana de recuento de CD4 basal y la prevalencia de diagnóstico tar-dío. Aplicamos la prueba de la suma de rangos de Wilcoxon para la variable contínua y la prueba de Fisher para com-parar proporciones.Resultados: Incluimos 199 personas (52 migrantes, 147 argentinos). Los migrantes presentaron un nivel basal de linfocitos CD4 significativamente menor [Mediana (RIC 25-75) 248 (79-466) vs. 331 (166-532); p=0,044], mayor tasa de presentación tardía [69,2% vs. 54,4%; RR 1,27 (IC95 1,01-1,61); p=0,072] y con sida [44,2% vs. 30,6%; RR 1,44 (IC95 0,98-2,13); p=0,089] y una menor proporción de diagnós-ticos en etapa temprana [13,5% vs. 29,3%; RR 0,46 (IC95 0,22-0,96); p=0,026].Conclusiones: Los migrantes internacionales accedieron al diagnóstico en peor estado clínico que los argentinos. Conocer este dato es imprescindible para elaborar políti-cas tendientes a mejorar el acceso al diagnóstico de esta población vulnerable.


Introduction: The late diagnosis of HIV infection and the access of patients to health system with advanced disease negatively affect the individual and collective benefits of antiretroviral treatment. There is a worldwide high prevalence of late diagnosis, specially in vulnerable populations, such as migrants.Objectives: to measure the prevalence of late diagnosis of HIV infection among international migrants and compare it with the people born in Argentina.Material and methods: retrospective and observational study of people over 16 years old, assisted at the Donación Francisco Santojanni General Hospital, who had received a diagnosis of HIV infection between 01-01-2018 and 12-31-2021. The median baseline CD4 cell count and the prevalence of late diagnosis were determined. We applied the Wilcoxon rank sum test for the continuous variable and the Fisher test to compare proportions.Results: 199 subjects (52 migrants, 147 Argentinians) were included. Migrants presented a significantly lower baseline CD4 cell count [Median (IQR 25-75) 248 (79-466) vs 331 (166-532); p=0.044], a higher rate of late presentation [69.2% vs 54.4%; RR 1.27 (CI95 1.01-1.61); p=0.072], presentation with aids [44.2% vs 30.6%; RR 1.44 (CI95 0.98-2.13); p=0.089]; and a lower proportion of early stage presentation [13.5% vs 29.3%; RR 0.46 (CI95 0.22-0.96); p=0.026].Conclusions: international migrants accessed to the diagnosis in a worse clinical condition than Argentinians. Knowing this information is essential for the development of policies aimed to improve the access to diagnosis of this vulnerable population

ABSTRACT

ABSTRACT People living with HIV (PLH), who use antiretroviral therapy (ART), are more susceptible to changes in the inflammatory profile and oxidative stress, and women have greater access to ART. Although physical exercise is a complementary strategy to treatment due to its antioxidant and anti-inflammatory effects, it is not clear whether acute responses to exercise can be harmful to PLH. The aim of the study was to investigate the acute effect of resistance exercise (RE) on inflammatory and oxidative stress markers in PLH. Ten women, using ART, performed RE session consisting of seven exercises for the whole body. For biochemical evaluation, blood samples were collected before (pre), 1 hour (1h) and 2 hours (2h) after the RE session. One-way ANOVA followed by Bonferroni's post hoc test was used to compare results between time points. There was an increase only in markers, GSSG of 160% (pre: 0.40 ± 0.11; 1h: 1.18 ± 0.36; 2h: 1.04 ± 0.25 mmol/g), TNF-α of 98 % (pre: 4.60 ± 0.55; 1h: 6.95 ± 0.77; 2h: 9.10 ± 1.03 pg/ml) and 52% IL-6 (pre: 2.47 ± 0 .67; 1h: 3.63 ± 1.26; 2h: 5.38 ± 2.15 pg/ml). The other variables remained unchanged (P > 0.05). It is concluded that a RE session increased the levels of inflammatory markers and oxidative stress in PLH in a non-exacerbated way.


RESUMO Pessoas vivendo com HIV (PVH), que utilizam a terapia antirretroviral (TARV), são mais suscetíveis a alterações no perfil inflamatório e estresse oxidativo, sendo que as mulheres possuem maior acesso à TARV. Embora o exercício físico seja uma estratégia complementar ao tratamento devido aos seus efeitos antioxidantes e anti-inflamatórios, não está claro se as respostas agudas ao exercício podem ser prejudiciais às PVH. O objetivo do estudo foi investigar o efeito agudo de exercícios com pesos (EP) sobre marcadores inflamatórios e de estresse oxidativo em PVH. Dez mulheres, em uso da TARV, realizaram uma sessão de EP constituída por sete exercícios para o corpo todo. Para avaliação bioquímica, amostras de sangue foram coletadas antes (pré), 1 hora (1h) e 2 horas (2h) após a sessão de EP. A ANOVA one-way seguida do teste post hoc de Bonferroni foi utilizada para comparação dos resultados entre os momentos. Houve aumento apenas nos marcadores, GSSG de 160% (pré: 0,40 ± 0,11; 1h: 1,18 ± 0,36; 2h:1,04 ± 0,25 mmol/g), TNF-α de 98% (pré: 4,60 ± 0,55; 1h: 6,95 ± 0,77; 2h: 9,10 ± 1,03 pg/ml) e IL-6 de 52% (pré: 2,47 ± 0,67; 1h: 3,63 ± 1,26; 2h: 5,38 ± 2,15 pg/ml). As demais variáveis permaneceram sem alterações (P > 0,05). Conclui-se que uma sessão de EP aumentou os níveis de marcadores inflamatórios e estresse oxidativo em PVH de forma não exacerbada.

ABSTRACT

The laboratory diagnostic strategy for human immunodeficiency virus (HIV) is a process to accurately detect HIV patients through a combination of available HIV tests. Laboratory tests for HIV infection are mainly serological antibody and antigen testing and HIV RNA testing. With the update of testing reagents, the sensitivity and specificity have improved substantially and the window period of detection has shortened, but there is a risk of false positives. Various guidelines have recommended different diagnostic strategies for different target populations and different prevalence regions to guide patients to confirm the diagnosis and receive standardized antiretroviral therapy as early as possible. How to refer to the diagnostic strategies, reduce false positives and shorten the window period while increasing the detection rate is an urgent issue for laboratories to address. This article describes the characteristics and advantages and disadvantages of testing methods related to HIV infection from the perspective of laboratory diagnostic strategies, as well as the impact of the development of treatments on diagnostic strategies, in order to provide theoretical support for the practical application of HIV diagnostic strategies.

ABSTRACT

Objective To propose the blood detection strategies for human immunodeficiency virus (HIV) among blood donors, and provide reference for the detection, early diagnosis and transmission blocking of HIV. Methods A total of 117 987 blood samples from blood donors were screened using the third- and fourth-generation ELISA HIV detection reagents. Western blot analysis was used to verify the reactive results of the third-generation reagent alone, or both the third-generation and fourth-generation reagents. HIV nucleic acid test was carried out for those with negative test results of the third- and fourth-generation reagents. For those with positive results of the fourth-generation reagent only, nucleic acid test followed by a confirmatory test by Western blot analysis was carried out. Results 117 987 blood samples from blood donors were tested by different reagents. Among them, 55 were tested positive by both the third- and fourth-generation HIV detection reagents at the same time, accounting for 0.047% and 54 cases were confirmed HIV-positive by Western blot analysis, and 1 case was indeterminate, then turned positive during follow-up testing. 26 cases were positive by the third-generation reagent test alone, among which 24 cases were negative and 2 were indeterminate by Western blot analysis. The band types were p24 and gp160 respectively detected by Western blot analysis, and were confirmed to be HIV negative in follow-up testing. 31 cases were positive by the fourth-generation HIV reagent alone, among which 29 were negative by nucleic acid test, and 2 were positive according to the nucleic acid test.Western blot analysis was used to verify that the two cases were negative. However, after 2~4 weeks, the results turned positive when the blood sample was retested by Western blot analysis during the follow-up of these two cases. All the specimens that were tested negative by both the third- and fourth-generation HIV reagents were validated negative by HIV nucleic acid test. Conclusion A combined strategy with both third- and fourth-generation HIV detection reagents can play a complementary role in blood screening among blood donors. The application of complementary tests, such as nucleic acid test and Western blot analysis, can further improve the safety of blood supply, thus contributing to the early diagnosis, prevention, transmission and treatment of blood donors potentially infected by HIV.

ABSTRACT

RESUMO Objetivo compreender as perspectivas e desafios no cotidiano de pessoas após a descoberta do viver com VIH em Bissau, Guiné-Bissau, tendo em vista diferentes contextos de vulnerabilidade. Método estudo exploratório-descritivo, que utilizou entrevista semiestruturada com 16 pessoas vivendo com VIH, acompanhadas em um hospital de Bissau. Empregou-se técnica de análise de conteúdo temática. Os relatos dos participantes foram analisados a partir de duas categorias empíricas: A descoberta, os impactos e os desafios de viver com VIH; e Experiência com o antirretroviral: recomeço e perspectivas. Resultados sinalizaram que os desafios iniciam com a revelação do diagnóstico que, geralmente, desperta uma diversidade de sentimentos e comportamentos. O estigma e a discriminação estimulam a adoção do sigilo sobre o status sorológico, resultando na fragilidade de suporte emocional no enfrentamento à soropositividade. A terapia antirretroviral foi vislumbrada como esperança para o enfrentamento da doença. A vulnerabilidade social foi a dimensão que mais se destacou, e violações dos direitos humanos foram constatadas. Conclusão e implicações para a prática o estudo permite compreender as perspectivas, desafios e vulnerabilidades de pessoas que vivem com VIH. O viver com VIH merece atenção especial por parte dos profissionais de saúde que atuam no cuidado dessas pessoas, destacando-se como contribuição a relevância de um cuidado de saúde integral, em que a ética e a subjetividade estejam presentes.


RESUMEN Objetivo este estudio cualitativo tuvo como objetivo comprender perspectivas y desafíos en la vida cotidiana de las personas que viven con VIH en Bissau, Guinea-Bissau, considerando diferentes contextos de vulnerabilidad. Método estudio exploratorio-descriptivo, que utilizó una entrevista semiestructurada con 16 personas que viven con el VIH, seguido en un hospital de Bissau. Se utilizó la técnica de análisis de contenido temático. Los informes de los participantes se analizaron a partir de dos categorías empíricas: El descubrimiento, los impactos y los desafíos de vivir con el VIH; y La experiencia antirretroviral: un nuevo comienzo y perspectivas. Resultados los resultados indicaron que los desafíos comienzan con la divulgación del diagnóstico, que generalmente suscita una diversidad de sentimientos y conductas. El estigma y la discriminación fomentan la adopción del secreto sobre el estado serológico, lo que resulta en un apoyo emocional débil para hacer frente a la seropositividad. La terapia antirretroviral es una esperanza para hacer frente a la enfermedad. La vulnerabilidad social fue la dimensión que más se destacó y se encontraron violaciones a sus derechos humanos. Conclusión e implicaciones para la práctica el estudio permite comprender las perspectivas, los desafíos y las vulnerabilidades de las personas que viven con el VIH. Vivir con VIH merece especial atención por parte de los profesionales de la salud que actúan en el cuidado de estas personas, destacando como aporte la relevancia de la atención integral en salud, en la que la ética y la subjetividad están presentes.


ABSTRACT Objective to understand perspectives and challenges in the daily lives of people after the discovery of living with HIV in Bissau, Guinea-Bissau, considering different contexts of vulnerability. Method an exploratory-descriptive study conducted through semi-structured interviews with sixteen people living with HIV attending a hospital in the city of Bissau. We used the thematic content analysis technique. Participants' reports were analyzed following two empirical categories: Discovery, impacts and challenges of living with HIV; and Experience with antiretroviral therapy: new beginning and perspectives. Results the results indicate that the challenges start with the disclosure of diagnosis, which arouses a diversity of feelings and behaviors. Stigma and discrimination encourage the adoption of confidentiality about serological status, which increases the fragility of emotional support in coping with seropositivity. Antiretroviral therapy was seen as a hope for coping with the disease. Social vulnerability was the dimension that stood out the most, and human rights violations involving people living with HIV were verified. Conclusion and implications for practice the study makes it possible to understand the perspectives, challenges and vulnerabilities of people living with HIV. Living with HIV deserves special attention from health professionals who work in the care of these people, highlighting as a contribution the relevance of comprehensive health care, in which ethics and subjectivity are present.

En Argentina se estima que 140 mil personas viven con VIH y de ellas el 17% no conocen su diagnóstico (Ministerio de Salud, 2021). La Dirección de Sida y Enfermedades de Transmisión Sexual (DSyETS) del Ministerio de Salud de la Nación realizó un estudio que mostró una prevalencia global de VIH de 2,68% en unidades del servicio penitenciario federal (DSyETS; 2017). Por ello nuestro objetivo fue favorecer el acceso al testeo y a la prevención de estas enfermedades en personas privadas de su libertad en una unidad penal de la provincia de Buenos Aires en el marco de la pandemia. Relato de experiencia: en diciembre del 2021 se ofreció el testeo voluntario, gratuito y confidencial para VIH y sífilis y accedieron 38 personas. Participaron de la actividad docentes, estudiantes del Departamento de Ciencias de la Salud de la Universidad Nacional del Sur y referentes del programa de VIH-ITS y HV de la Región Sanitaria I del ministerio de salud de la provincia de Buenos Aires. Conclusiones: Esta experiencia mostró la importancia de construcción de redes para la articulación de prácticas que favorezcan el acceso a un diagnóstico temprano y tratamiento oportuno para VIH y sífilis a las personas viviendo en contexto de encierro (AU)


In Argentina, it is estimated that 140 thousand people live with HIV and 17% of them do not know their diagnosis (Ministry of Health, 2021). The Directorate of AIDS and Sexually Transmitted Diseases (DSyETS) of the Ministry of Health of the Nation carried out a study that showed a global prevalence of HIV of 2.68% in units of the federal prison service (DSyETS; 2017). For this reason, our objective was to promote access to testing and the prevention of these diseases in people deprived of their liberty in a penal unit in the province of Buenos Aires in the context of the pandemic. Experience report: in December 2021, voluntary, free and confidential testing for HIV and syphilis was offered and 38 people agreed. Teachers, students from the Department of Health Sciences of the National University of the South and referents of the HIV-STI and HV program of the Sanitary Region I of the Ministry of Health of the province of Buenos Aires participated in the activity. Conclusions: This experience showed the importance of building networks for the articulation of practices that favor access to early diagnosis and timely treatment for HIV and syphilis for people living in a confinement context (AU)

ABSTRACT

OBJETIVO: Caracterizar por sexo una población universitaria que accedió voluntariamente a una prueba de detección rápida del virus de la inmunodeficiencia humana (VIH). MÉTODO: Estudio analítico de 3864 universitarios/as. Variables: sociodemográficas y académicas, comportamiento sexual, prevalencia de infecciones de transmisión sexual (autorreporte) y motivos para realizarse el test rápido. RESULTADOS: El 61,4% son mujeres, edad media 21,64 años, edad de inicio de la actividad sexual 17,01 años. Actividad sexual casual: 36,8%, destacando los hombres (p < 0,001). Sexo anal: 44,5%. Prevalencia de infecciones de transmisión sexual: 7,5%, destacando los hombres en gonorrea (p < 0,001) y sífilis (p = 0,032). Test rápido VIH (+): 0,2%. Consumo de sustancias durante la actividad sexual: 50%. El 41,1% nunca pregunta a su contacto sexual el estado serológico de VIH, destacando los hombres (p < 0,001), y el 64,4% no sabe si ha tenido una pareja sexual VIH (+), destacando los hombres (p < 0,001). El 16,5% siempre ha usado preservativo, destacando las mujeres (p = < 0,001). El 26,21% usa redes sociales para encuentros sexuales, con diferencias por sexo. CONCLUSIONES: Las mujeres demandan más el test rápido de VIH que los hombres. Existen conductas de riesgo y diferencias por sexo: poco uso del preservativo, relaciones sexuales casuales y bajo la influencia de sustancias, prácticas sexuales anales y orales sin protección, desconocer el estado serológico de su contacto sexual y uso de redes sociales para encuentros sexuales.


OBJECTIVE: To characterize a university population that willingly took a rapid HIV test by sex. METHOD: An analytical study with 3,864 university students. Variables: socio-demographic and academic variables, sexual behavior, (self-reported) STI prevalence, and reasons to take the rapid test. RESULTS: 61.4% are women; average age: 21.64 years; age of first sexual intercourse: 17.01 years; casual sexual activity: 36.8%, mostly men (p < 0,001), anal sex: 44.5%; STI prevalence: 7.5%, with gonorrhea and syphilis being more common among men (p < 0.001 and p < 0.032, respectively; positive rapid HIV test: 0.2%; substance use during sex: 50%. 41.1% of the participants, most of them men (p < 0.001), have never asked their partner about their serologic HIV status, and 64.4% does not know if they have had a HIV+ partner, an attitude that is more prevalent among men (p < 0.001). 16.5% has always used prophylactics, mostly women (p < 0.001). 26.21% uses social networks to arrange sexual encounters. CONCLUSIONS: Women ask more for the rapid HIV test than men. There are several risky behaviors and gender differences: low use of prophylactics, casual sexual relationships under the influence of substances, unprotected anal and oral sex practices, ignorance of their sexual partners serologic status, and the use of social networks for sexual encounters.

RESUMO Objetivo: construir e validar um cenário de simulação clínica sobre a testagem rápida e aconselhamento para o HIV em gestantes. Método: estudo metodológico, de validação de aparência e conteúdo, desenvolvido no período de junho a outubro de 2020 por meio da técnica Delphi. Para a validação, foram incluídos os juízes que obtiveram cinco ou mais pontos segundo os critérios de Fehring adaptados. Os dados foram analisados mediante o cálculo do Índice de Validade de Conteúdo (IVC). Resultados: depois da primeira rodada Delphi, dois itens (5,7%) não atingiram o I-CVI necessário para validação em todos os critérios avaliados, que foram: comportamental, objetividade, simplicidade, clareza, relevância, precisão, variedade, modalidade, tipicidade e credibilidade. Ao final da segunda rodada Delphi, todos os itens (100%) atingiram o I-CVI necessário para validação. Conclusão: o roteiro se mostrou válido, contribuindo para subsidiar o ensino da testagem e do aconselhamento de gestante sobre o HIV.

Method: methodological study, of appearance and content validation, developed between June and October 2020 through the Delphi technique. For validation, the judges who obtained five or more points according to the adapted Fehring criteria were included. The data were analyzed by calculating the Content Validity Index (CVI). Results: After the first Delphi round, two items (5.7%) did not reach the I-CVI required for validation in all the criteria evaluated, which were: behavioral, objectivity, simplicity, clarity, relevance, accuracy, variety, modality, typicality, and credibility. At the end of the second Delphi round, all items (100%) reached the I-CVI required for validation. Conclusion: the script proved to be valid, contributing to subsidize the teaching of HIV testing and counseling of pregnant women.

Resumen Desde la llegada de la pandemia por COVID-19, se ha descrito un sinnúmero de consecuencias respecto a la infección por VIH en Chile, tanto en su diagnóstico como en su prevención, principalmente manifestadas en el descenso de la realización de exámenes serológicos, la interrupción de la atención, el retraso en el inicio de la terapia antirretroviral y la falta de acceso a métodos preventivos. A ello, se le suma el deterioro en algunas condiciones relacionadas con aspectos sociales y sobre salud mental. Todo esto implica, entre otras consecuencias, un probable empeoramiento en la situación epidemiológica de la infección por VIH en Chile. En el presente artículo, se revisarán algunos de los impactos señalados y se plantean algunos desafíos, para el enfrentamiento de esta problemática.


Abstract Since the arrival of the COVID-19 pandemic, countless consequences have been described regarding HIV infection in Chile, both in its diagnosis and prevention, mainly manifested in the decrease in serological tests, the interruption of care, the delay in starting antiretroviral therapy, and the lack of access to preventive methods. Added to this is the deterioration in some conditions related to social aspects and mental health. All this implies, among other consequences, a probable worsening in the epidemiological situation of HIV infection in Chile. Some of the designated impacts are reviewed in this article, and some challenges are raised to confront this problem.

ABSTRACT

SUMMARY OBJECTIVE: The objective of this study was to emphasize the importance of legal and bioethical knowledge in maintaining medical confidentiality, especially in situations when there is a diagnosis of HIV infection. METHODS: A literature review of studies published in the Scientific Electronic Library Online and National Library of Medicine databases was performed. Sixteen studies available in full, online, and free, published between 2010 and 2020, were selected. RESULTS: The studies highlighted that, despite the ethical duty to breach confidentiality for the protection of third parties, many doctors are reluctant to reveal this secret due to the power of stigmatization and social discrimination related to the diagnosis of HIV infection, which affects integrity, counseling, and capability to treat patients. CONCLUSION: HIV diagnosis implies bioethical and legal questions. Respect for medical confidentiality is a matter to be discussed, as there is a need to protect the privacy of the patient, at the same time the responsibility to preserve the health of others.

Introducción: El virus de inmunodeficiencia humana (VIH) causa una infección que destruye los linfocitos del huésped. Su fase avanzada conlleva al Síndrome de Inmunodeficiencia Adquirida (SIDA) que trae consigo una alta morbimortalidad a nivel mundial, convirtiéndose en una pandemia de gran preocupación para las organizaciones mundiales de salud. Materiales y Métodos: Estudio de carácter observacional, descriptivo, ecológico y longitudinal retrospectivo. Se analizaron los datos del Departamento de Estadísticas de Información de Salud, número de casos confirmados, defunciones ocurridas en la población chilena, por región, entre los años 2016 y 2021 con causa básica de muerte Enfermedad por VIH [SIDA] y la cantidad total de exámenes realizados. Resultados: Se observa un aumento tanto en el número de exámenes realizados como de los casos diagnosticados hasta el año 2019. Respecto a la mortalidad se describe una tendencia decreciente desde el año 2016, con un alza puntual en 2019 tanto a nivel nacional como regional, por rango etario y sexo. A nivel regional se identifican mayores cifras en las macrozonas norte y centro. Discusión: En todas las regiones se evidenció una disminución durante los años 2020 y 2021, posiblemente asociada a un camuflaje en su categorización. La población chilena presenta una mayor tasa de mortalidad en el rango de 40 a 44 años, a diferencia del contexto mundial donde se observa un rango de 55 a 59 años. En causas específicas de mortalidad, se destaca el segundo lugar asociado a tumores malignos siendo importante destacar la necesidad de evidencia enfocada en el diagnóstico temprano de estas patologías.


Introduction: Human immunodeficiency virus (HIV) causes an infection that destroys host lymphocytes. Its advanced stage leads to Acquired Immunodeficiency Syndrome (AIDS) that brings with it a high morbimortality worldwide, becoming a pandemic of great concern for world health organizations. Materials and Methods: Observational, descriptive, ecological and retrospective longitudinal study. Data from the Department of Health Information Statistics, number of confirmed cases, deaths occurred in the Chilean population, by region, between the years 2016 and 2021 with basic cause of death HIV disease [AIDS] and the total number of tests performed were analyzed. Results: An increase is observed both in the number of examinations performed and cases diagnosed until 2019. Regarding mortality, a decreasing trend is described since 2016, with a punctual increase in 2019 both at national and regional level, by age range and sex. At the regional level, higher figures are identified in the northern and central macro-zones. Discussion: A decrease was evident in all regions during 2020 and 2021, possibly associated with a camouflage in their categorization. The Chilean population presents a higher mortality rate in the range of 40 to 44 years, unlike the world context where a range of 55 to 59 years is observed. In specific causes of mortality, the second place associated with malignant tumors stands out, being important to highlight the need for evidence focused on the early diagnosis of these pathologies.

ABSTRACT

O sarcoma de Kaposi é uma neoplasia maligna associada à infecção pelo herpes vírus humano 8 em doentes imunossupressos. O sarcoma de Kaposi Epidêmico é o tipo epidemiológico mais frequente e afeta indivíduos VIH-positivos. A região anoperineal é raramente envolvida e as lesões suspeitas devem ser biopsiadas para confirmação histológica. A base do tratamento é a restauração imune do doente. Relatamos o caso de um jovem, com diagnóstico recente de infeção pelo VIH, sem tratamento, que foi admitido no serviço de infectologia apresentando sintomas constitucionais, adenomegalias inguinais e extensa lesão verrucosa e ulcerada na região anoperineal. As biópsias confirmaram o diagnóstico de sarcoma de Kaposi e o doente iniciou terapia antirretroviral e quimioterapia. Houve recuperação clínica, regressão das lesões e desaparecimento das adenomegalias. Este relato objetiva alertar as equipes médicas no sentido de se incluir o sarcoma de Kaposi no diagnóstico diferencial das lesões que afetam a região anoperineal.


Kaposi's sarcoma is a malignant neoplasm associated with human herpesvirus 8 infection in immunocompromised patients. Epidemic Kaposi's sarcoma is the most common epidemiological type and affects HIV-positive patients. Perineal involvement is rare, and suspicious lesions should be biopsied to confirm histological diagnosis. Treatment consists of restoring the patient's immune system. We report the case of a young patient recently diagnosed with HIV, without treatment, who was admitted to the Department of Infectious Diseases with nonspecific symptoms, inguinal lymphadenopathy, and an extensive verrucous ulcerated lesion in the perineal region. Biopsy confirmed the diagnosis of Kaposi's sarcoma, and the patient was started on antiretroviral therapy and chemotherapy. Clinical recovery was achieved, with lesion reduction and inguinal adenopathy resolution. This case report aims to encourage physicians to include Kaposi's sarcoma in the differential diagnosis of perineal lesions.

O objetivo do estudo foi descrever o conhecimento e práticas de risco à infecção pelo HIV na amostra total de cada município, entre homens de 15 a 24 anos que vivem sem companheiro(a), e homens que fizeram sexo com homems (HSH) pelo menos uma vez na vida em três cidades brasileiras. Foi realizado estudo de corte transversal de base domiciliar com amostragem por conglomerados em três estágios (setores censitários, domicílios, indivíduos), com estratificação por sexo, faixa etária (15-24; 25-34; 35-44; 45-59) e vive com companheiro(a) na seleção do indivíduo. Estimaram-se proporções e intervalos de 95% de confiança (IC95%) de indicadores de conhecimento, testagem do HIV, comportamento sexual e autoavaliação do risco. Foram analisados 5.764 indivíduos em Campo Grande, 3.745 em Curitiba e 3.900 em Florianópolis. Baixo nível de conhecimento foi encontrado para os métodos de prevenção, sobretudo para profilaxia pré-exposição (PrEP). Práticas de sexo desprotegido foram frequentes nos três municípios. As proporções de teste de HIV na vida foram 57,2% (IC95%: 55,1-59,2) em Curitiba, 64,3% (IC95%: 62,7-66,0) em Campo Grande, e 65,9% (IC95%: 64,0-67,7) em Florianópolis. Entre homens de 15-24 anos, proporções de uso de drogas estimulantes e práticas sexuais desprotegidas foram mais altas que nos demais grupos etários. Entre os HSH, as proporções de teste de HIV na vida foram superiores a 80%. Mais de 30% foram parceiros receptivos no sexo anal sem uso de preservativo, e menos de 5% avaliam seu risco como alto. É preciso adotar estratégias de comunicação mais eficazes sobre a prevenção da infecção do HIV, incluindo a ampliação de conhecimentos que poderiam motivar práticas sexuais mais seguras.

The study aimed to describe knowledge and risk practices related to HIV infection in three Brazilian cities in the general population, men 15 to 24 years of age living without a partner, and men that reported sex with other men (MSM) at least once in life. This was a cross-sectional household-based study with three-stage cluster sampling (census tracts, households, individuals) stratified by sex, age group (15-24; 25-34; 35-44; 45-59), and conjugal status in the individual selection. We estimated the proportions and 95% confidence intervals (95%CI) of indicators of knowledge, HIV testing, sexual behavior, and self-rated risk. We analyzed 5,764 individuals in Campo Grande, 3,745 in Curitiba, and 3,900 in Florianópolis. Low levels of knowledge were found for preventive methods, especially PrEP. Unprotected sex practices were frequent in the three municipalities. Lifetime HIV test rates were 57.2% (95%CI: 55.1-59.2) in Curitiba, 64.3% (95%CI: 62.7-66.0) in Campo Grande, and 65.9% (95%CI: 64.0-67.7) in Florianópolis. Among men 15-24 years of age, the proportions of stimulant drug use and unprotected sexual practices were higher than in the other age groups. Lifetime HIV test rates exceeded 80% in MSM. More than 30% of MSM were receptive partners in anal sex without condoms, and fewer than 5% assessed their risk as high. More effective communication strategies are needed on prevention of HIV infection, including increased knowledge that could motivate safer sexual practices.

ABSTRACT OBJECTIVE Recognize incident infection to better characterize the groups that fuel HIV epidemic. We propose a simple score to identify recent infections among newly diagnosed patients as a HIV surveillance tool. METHODS Newly diagnosed patients were defined as recent infections when a negative serological test in the previous year was available. Laboratory tests, such as the avidity index (Bio-Rad, according to the CEPHIA protocol), chemiluminescent intensity (CMIA, architect, Abbott), and the nucleotide ambiguity index of partial pol sequences were used as proxies of recency. A simple score based on clinical symptoms of acute retroviral syndrome during the previous year, CD4+ T cell count, and viral load at admission was tested to assess the predictive power, using receiver operating characteristic (ROC) curves, to identify recent cases of infection. RESULTS We evaluated 204 recently diagnosed patients who were admitted to the Ambulatório de Referência em Moléstias Infecciosas de Santo André (Santo André Reference Infectious Diseases Outpatient Clinic), in the metropolitan region of São Paulo, Brazil, recruited between 2011 and 2018. An HIV-negative test in the year prior to enrollment was documented in 37% of participants. The proportion of cases classified as recent infections (less than one year), according to the laboratory proxies were: 37% (67/181) for an avidity index < 40%, 22% (30/137) for a CMIA < 200, and 68% (124/181) for an ambiguity index < 0.5%. Using different combinations of recency definitions, our score showed an area under the ROC curve from 0.66 to 0.87 to predict recency. CONCLUSIONS Using data from patients' interviews and routine laboratory tests at admission, a simple score may provide information on HIV recency and thus, a proxy for HIV incidence to guide public policies. This simple for the Brazilian public health system and other low- and middle-income countries.

Objetivo: analisar o discurso dos gerentes de saúde a respeito das potencialidades para realização do Teste Rápido anti-HIV na Atenção Primária à Saúde. Método: estudo exploratório, qualitativo, realizado em um dos cinco Distritos Sanitários da capital paraibana. A coleta de dados foi realizada em setembro de 2017. Utilizou-se o dispositivo teórico-metodológico da Análise do Discurso, de matriz francesa, por meio do conceito-análise potencialização da APS na consolidação do TR anti-HIV, divididos em dois blocos discursivos: a acessibilidade geográfica e a acessibilidade sócio-organizacional. Resultados: os discursos apontam elementos potencializadores da APS para a realização TR anti-HIV: proximidade do serviço de saúde, arranjos organizacionais, diagnóstico precoce do HIV, agilidade no resultado do teste rápido anti-HIV e tratamento imediato. Conclusão: verificou-se potencialidades na consolidação do teste rápido anti-HIV, evidenciando a Atenção Primária à Saúde como um espaço facilitador na ampliação da integralidade e o acesso aos serviços de saúde.


Objective: to analyze the discourse of health managers regarding the potentialities for conducting the HIV Rapid Test in Primary Health Care. Method: exploratory, qualitative study, conducted in one of the five Health Districts of the capital of Paraíba. Data collection was carried out in September 2017. We used the theoretical and methodological device of Discourse Analysis, of French matrix, through the concept-analysis potentialization of PHC in the consolidation of anti-HIV TR, divided into two discursive blocks: geographical accessibility and socio-organizational accessibility. Results: the speeches point out potentializing elements of PHC for the realization of anti-HIV test: proximity of the health service, organizational arrangements, early diagnosis of HIV, agility in the result of the rapid anti-HIV test and immediate treatment. Conclusion: potentialities in the consolidation of the anti-HIV rapid test were verified, evidencing Primary Health Care as a facilitating space in the expansion of integrality and access to health services.

Resultados: los discursos muestran los elementos potenciales de la APS para la realización de TR contra el VIH: la proximidad del servicio de salud, los arreglos organizativos, el diagnóstico precoz del VIH, la agilidad en el resultado de la prueba rápida contra el VIH y el tratamiento inmediato. Conclusión: se verificaron las potencialidades en la consolidación de la prueba rápida contra el VIH, evidenciando a la Atención Primaria a la Salud como un espacio facilitador en la ampliación de la integralidad y el acceso a los servicios de salud.

Introduction: There is a need for studies to know the real situation and outline measures to guarantee a reduction in the rates of pregnant women diagnosed with HIV, Syphilis and Toxoplasmosis. Objective: To determine the prevalence of Syphilis, HIV and toxoplasmosis in puerperal women assisted at the largest public maternity hospital in Campos dos Goytacazes in 2016. Methods: Cross-sectional study, using secondary data from the puerperal women assisted at the largest maternity hospital in the northern region of the State of Rio de Janeiro, in the year 2016. Results: There were 970 puerperal women, with a prevalence of HIV, Syphilis and Toxoplasmosis in pregnancy of 1.6, 2.7 and 2%, respectively. Most pregnant women were diagnosed at delivery due to low serological coverage during pregnancy. Conclusion: The high prevalence of Syphilis, HIV and Toxoplasmosis in pregnancy requires efficient prenatal care for its identification and approach.


Introdução: São necessários estudos para conhecer a real situação e delinear medidas que garantam a redução dos índices de gestantes com diagnóstico de vírus da imunodeficiência humana, sífilis e toxoplasmose. Objetivo: Determinar a prevalência de sífilis, vírus da imunodeficiência humana e toxoplasmose em puérperas atendidas na maior maternidade pública de Campos dos Goytacazes (RJ) no ano de 2016. Métodos: Estudo transversal, utilizando dados secundários das puérperas atendidas na maior maternidade do Norte Fluminense no ano de 2016. Resultados: Foram 970 puérperas, com prevalência de vírus da imunodeficiência humana, sífilis e toxoplasmose na gestação de 1,6, 2,7 e 2%, respectivamente. A maioria das gestantes foi diagnosticada no momento do parto em razão da baixa cobertura sorológica durante a gestação. Conclusão: A alta prevalência de sífilis, vírus da imunodeficiência humana e toxoplasmose na gravidez requer um pré-natal eficiente para sua identificação e abordagem.

INTRODUCCIÓN: La Organización Mundial de la Salud y la Académica Americana de Pediatría sugieren revelar el diagnóstico de VIH a niños. Sin embargo, esto no es una práctica consolidada en Colombia; a menudo esto ocurre porque los cuidadores no tienen una metodología adecuada para llevar a cabo este proceso. OBJETIVO: Se presenta la evaluación de la eficacia de la historia "Mi Dragón Dormido" para el proceso de educación y revelación del diagnóstico de VIH/SIDA a niños. MÉTODO: Mediante muestreo no probabilístico intencional; se administraron entrevistas semiestructuradas a 40 niños, seropositivos (n = 20) y seronegativos (n = 20) para VIH/SIDA. Se trató de un estudio cualitativo y diseño sistemático. Se realizó análisis cualitativo mediante el software NVivo 10 con codificación tipo Nodos de Árbol. RESULTADOS: Los datos cualitativos aportaron aproximaciones necesarias para mejorar el contenido y el diseño de la herramienta e indicaron que refleja la realidad de los niños y permite identificar temores y preguntas frecuentes frente a la condición crónica de salud del VIH. CONCLUSIONES: Estos resultados aportan información importante a considerar en el proceso de revelación del diagnóstico y su manejo por parte del personal de salud.


BACKGROUND: The World Health Organization and the American Academy of Pediatrics suggest disclose the HIV diagnosis to children. However, the disclosure of HIV/AIDS diagnosis to children and adolescents, it is not a consolidated practice in Colombia, often this occurs because caregivers do not have an adequate methodology to carry out this process. AIM: We present the efficiency components validation of "Mi dragon Dormido" history in the educational process and revelation ofAIDS diagnosis to boys and girls. METHOD: By means of purposive non-probability sampling, semi-structured interviews were conducted with 40 children, with an equal number of children who were seropositive (n = 20) and seronegative (n = 20) for HIV/ AIDS. A qualitative study with the approach of grounded theory and systematic design was conducted. We conducted the qualitative study through the NVivo software, with the codification type being Tree Nodes. RESULTS: The qualitative data provided necessary approximations to improve the content and design of the tool and showed that it reflects the reality of children and allows them to identify frequent fears and questions regarding to chronic health condition of HIV. CONCLUSIONS: These results provide important information to be considered in the process of diagnosis revelation and its management by health personnel.
